Job Summary
The Registered Nurse IIC (Care Facilitator) is a registered nurse who functions in a leadership capacity, coordinating the daily activities of the unit, including admissions, discharge and transfer of patients. The Care Facilitator will be accountable for the continuity and coordination of patient care utilizing clinical judgment and critical thinking skills; assist with the development, implementation and evaluation of quality and patient safety initiatives; and ensure a high standard of patient care is maintained. The Care Facilitator will evaluate the effectiveness of patient care through such mechanisms as team conferences, ongoing feedback with team members/patients/family, and review of staff assignments. The Care Facilitator also participates as an active team member and acts as a liaison for team members in the interdisciplinary provision of care; facilitates and coordinates patient/family care with other disciplines, departments and programs; and performs other related duties as required The Care Facilitator is accountable for assisting the Division Manager to implement continuous quality initiatives through education, auditing & feedback mechanisms. This Care Facilitator Role, being set in Rural Avalon, has a significant role in relation to educational support for nursing staff. The Care Facilitator will provide leadership in relation to educational needs of staff specific to Labor & Delivery as well as Surgical Services providing direct education as well as coordination of education, evaluation, and monitoring of staff attendance & compliance.

Regularly consults/interacts with other departments, units, and hospitals to coordinate timely patient/client/resident flow and to communicate regarding division/unit issues.
Regularly stands or walks to perform audits, give presentations, respond to patient/client/resident complaints and collaborate with the health care team to resolve issues.
Regularly performs fine finger/precision work on the computer or with machines that require accurate control and steadiness.
Visual concentration is required to view and report on documents, assess workplace situations, and/or to monitor the workplace regarding health, safety and risk management issues.
Job Qualifications
Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate degree in Nursing or equivalent education or training is required.
Current practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required.
Current BLS Certification is required.
Minimum of five (5) years' experience within the last seven (7) in case room is required.
Demonstrated competency in organizational, leadership, interpersonal, team-building, communication, teaching and decision-making skills.
Demonstrated initiative and ability to work independently.
Demonstrated progressive leadership.
Evidence of active involvement or willingness to participate in continuing professional education development.
Experience in facilitating staff and student clinical initiatives.
Membership in professional organizations/special interest groups is an asset.
Knowledge of continuous quality improvement concepts.
A satisfactory record of work performance and attendance is required.
